I am the owner or agent for the owner of the above-described animal and have the authority to execute this consent. I hereby consent and authorize Apalachee River Animal Hospital to perform the above procedure (s)* I understand that during the performance of the aforementioned procedure(s) or operation(s), unforeseen conditions may occur that necessitate an extension of the aforementioned or different measures than those set forth above. Therefore, I hereby consent to and authorize the performance of such procedure(s) or operation(s) as are necessary and desirable in the exercise of the veterinarian’s professional judgment. I also authorize the use of appropriate anesthetics and other medications. I understand that hospital support personnel will be employed as deemed necessary by the veterinarian. I have been advised as to the nature of these procedure(s) and/or operation(s) and the risks involved. I agree to pay all costs associated with the aforementioned. I agree to pay any amounts that are assessed by virtue of this account being sent to a collection agency. Pre-Surgical Blood Work(Required)
Each patient presented for anesthesia and surgery will receive a pre-anesthetic physical exam. Some animals, however, may have underlying conditions or diseases that can only be detected by screening blood tests. Therefore, we require that all patients undergoing anesthesia and surgery have routine blood work performed. These tests are required for all patients undergoing elective (spays, neuters, dental cleanings, etc.) procedures and non-elective procedures. The cost for this testing is $75.98.
Microchipping(Required)
Permanent identification microchipping, which can provide a quick and easy reunion between pet and owner in the event a pet becomes lost, is available while your pet is under anesthesia. The cost for microchipping is $53.50.
Elizabethan Collar:(Required)
Your pet may be sent home with an elizabethan collar (e-collar) to use after his/her procedure here at the hospital. Please keep the collar on at all times, to allow incisions or other affected areas to heal.
